
Founded in 1984, Senior Care Network (SCN) has carved a unique niche as one of the nation’s few hospital-based programs specifically designed to meet the diverse needs of older adults, adults with disabilities, and their families.
The SCN organization’s team, which includes social workers, public health nurses, volunteers, and students, also partners with physicians, mental health and community-based providers, and health plans to address social determinants of health. Together, they serve more than 15,000 community members annually, with the goal of helping people age well and live safely in their homes.

“Our dedicated team works closely with individuals and families every day, personally answering questions to help them navigate through the health-related challenges of aging. This personalized approach has supported thousands of older adults and people with disabilities to live safely and continue to thrive in the community.”
SCN offers a range of free programs and support services, including a 50+ Health Connection newsletter, Assisted Living Waiver Program (ALW), caregiver support, call-in community resource center, the Senior CareLine, and the Multipurpose Senior Services Program (MSSP). The organization provides support for all socioeconomic levels: Medicare, Medi-Cal, dual-eligible, homeless, middle-income and wealthy, and accommodates various language needs.
Funding for Senior Care Network comes from a combination of public programs, community donations, and proceeds from the Huntington Collection resale shop. This enables the organization to provide personalized support to thousands of older adults and people with disabilities, helping them continue to thrive in the community.
